Clinical Consequences of Portal Hypertension

چکیده
The portal hypertensive syndrome is responsible for many of the manifestations of advanced, decompensated l iver disease. Some of these complications are a direct consequence of portal hypertension: gastrointestinal bleeding result ing from ruptured gastroesophageal varices, PHG, and colopathy; hyperkinetic syndrome; hypersplenism; and an increased systemic availabil i ty of drugs and endogenous compounds with rapid hepatic uptake (5). In other complications, portal hypertension plays a key role, although it is not the only pathophysiologic factor in their development. These include ascites, abnormalit ies of renal function, hepatorenal syndrome, spontaneous bacterial peritonit is, hepatopulmonary syndrome, and hepatic encephalopathy. This chapter focuses on gastrointestinal bleeding; the other complications are dealt with elsewhere in this book.

Hepatoportal sclerosis or idiopathic portal hypertension has a worldwide distribution with prevalence in developing and temperate countries. Of 64 patients with this disease seen during a twelve year period, 36 underwent splenectomy or a shunt procedure. The indications for surgical intervention were severe hypersplenism and persistent left upper abdominal pain andlorhistory of frequent ep...
متن کاملCurrent management of the complications of portal hypertension: variceal bleeding and ascites.
Portal hypertension is one of the main consequences of cirrhosis. It results from a combination of increased intrahepatic vascular resistance and increased blood flow through the portal venous system. The condition leads to the formation of portosystemic collateral veins. Esophagogastric varices have the greatest clinical impact, with a risk of bleeding as high as 30% within 2 years of medium o...
متن کاملThe kidney in cirrhosis with portal hypertension
Ascites, dilutional hyponatremia and hepatorenal syndrome are three clinical manifestations of the same physiopathological disorder: cirrhotic portal hypertension, hyperproduction of nitric oxide, arterial vasodilation with reduction of efficient arterial volume, which have as consequences renal vasoconstriction, sympathetic stimulation, the stimulation of renin-angiotensin-aldosteron system an...
متن کاملMinocycline attenuates cirrhotic cardiomyopathy and portal hypertension in a rat model: Possible involvement of nitric oxide pathway
Objective(s): An increase in nitric oxide (NO) production has been reported in cirrhotic cardiomyopathy and, portal hypertension. Since minocycline has been shown to inhibit NO overproduction, we aimed to examine its role in a rat model of CCl4-induced cirrhotic cardiovascular complications. Materials and Methods: Portal pressure and inotropic responsiveness of isolated papillary muscles to is...
